About Our Services:

Disability Management Services
Disability Management Services was developed to provide you with the tools necessary to develop and implement a successful return to work program for injured or ill employees who may be limited in their ability to maintain productivity due to physical or emotional complications. A variety of services are offered, including: Consultation/Education Services, Post Offer/Pre-Employment Services, and Case Management/Prevention Services.

Medical Management
A rehabilitation consultant conducts on-site, face-to-face meetings with an injured worker, the employer and the healthcare provider.  Our goal is to assist the injured worker through the maze of medical treatment, assure appropriate medical care and return the employee to work.

Vocational Evaluation and Assessment
When injured employees cannot return to work in their original positions, a rehabilitation consultant determines their potential for new ones.  An injured worker’s physical, vocational and cognitive abilities are assessed to determine how they may be used in a new position.  Vocational rehabilitation can include vocational assessment, vocational testing, transferable skills analysis, job analysis, surveying the labor market, teaching job seeking skills, identifying potential employers, interviewing and following up with employers. 

Job Development and Placement
Vocational case manager works individually with claimant to develop and provide job leads and placement. Also provides follow up and documentation of compliance.

IME Only
Carolina Case Management will coordinate and schedule an IME with appropriate physician.  We will send a letter to the physician confirming the IME and listing requested information to be addressed by the physician.  A letter will also be sent to the client to confirm the IME.  The physician's office will also be contacted for confirmation.  With client approval, we will attend the IME appointment to get the needed information from the client and the physician.  A report will be generated providing the requested information and the file will be closed unless the carrier requests further activity.  

Telephonic Case Management
Includes file assignment to a specified Registered Nurse for complete medical case management services via telephone. This type of case management is a very cost-effective tool for claims that are not complex enough to require extensive onsite medical management.

Labor Market Survey
Conducted to determine viability of employment in a particular geographic area. Carolina Case Management will contact prospective employers whose work environments match the client’s vocational abilities and physical restrictions to determine job availability and salary levels. A report with recommendations will be provided and the file will be closed unless further activity is requested by the carrier.

Medical Bill Audits
Review of a file and/or bill to render a nursing opinion on such issues as appropriateness of treatment to claimant’s diagnosis or accident and subsequent related costs.

Utilization Review
Encompasses review of inpatient and outpatient procedures and treatments; and may involve reviews by physicians, not associated with Carolina Case Management, to address specific issues.

Peer Review
Carolina Case Management will arrange to have a specialized physician review medical records and diagnostic studies to determine appropriateness of care or proposed plan of treatment. A full report will follow the evaluation.

Life Care Planning
We will review the current status and history of the referred patient and will research to determine the approximate lifetime costs of medical care. This information is extremely useful in setting reserves for catastrophic cases.

National Network
By establishing strategic alliances with privately owned rehabilitation companies possessing compatible philosophies and quality assurance standards, the National Network is capable of offering consistency of service and results throughout the United States.  We believe that utilization of this approach is superior to that of national firms.  Through the National Network, customers can now utilize the "local" companies known for excellent, personalized service for all their rehabilitation needs.

Customized Services
In addition to the standard services described here, Carolina Case Management will make every effort to accommodate individual needs through customized programs and services. We work with the carrier to define, design, and implement these custom services. We will also provide a Value Statement periodically to show your true cost containment savings.

Medical Community Relationships
During our many years experience in this field, we have developed close working relationships with many of the state’s top physicians and treatment facilities. We have negotiated rate reductions for many treatment sources such as HealthSouth and NovaCare Rehabilitation. We have established numerous vendor contacts for medical equipment and services. Isn’t it nice to know these savings are directly related to your cost savings?

Carolina Eldercare, Inc. was formed in 2003 in response to the growing need for care management services for older adults. As the number of elderly citizens in America continues to grow, so too do the needs for support services.
